If you are not of our faith then still complete the form as best you can. We will fully consider honest God fearing applications from other denominations.

Therefore you are required to complete and return the enclosed form with all due haste considering your health. This form may be filled on your behalf by your legal representatives or somebody known and trusted by you. Once that is done to our church committee's satisfaction and our prayers have been received and accepted by The Blessed Virgin Mary we can proceed with accepting your most generous donation to be used in the Lords work. Amen

In handling your donation we only ask that our expenses are covered plus an administrative fee of 10% of any money expended.
